OFFICE VISITS
Before Coming
- Please download & fill out our Client Health History Form prior to your visit. You can also download, print & fill all of our other forms by visiting our Forms Page.
- Please bring your insurance provider card.
1st Visit - What To Expect
During your first visit, we will review your dental and medical history forms. Take X-rays of your teeth and mouth. Then, the doctor will meet with you to discuss your X-rays, help you create your own individual treatment plan, and answer any questions that you may have.
2nd Visit - What To Expect
On your follow-up visit, one of our hygienists will clean and polish your teeth, checking your teeth and gums for any cavities or early signs of tooth decay or gingivitis. Discuss future procedures, answer questions and help with any insurance provider or financial questions you may have.
